What Is MDVIP?
MDVIP is a member-based service. It is a wellness plan that puts you the patient first to provide the highest level of personal care. Members enjoy benefits such as 24/7 availability to the provider, same-day or next-day appointments that start on time, a customized wellness plan that addresses diet and exercise, and a doctor who truly knows you. With these benefits and more, MDVIP transforms the traditional healthcare experience to offer invaluable personalized care.
Does my insurance cover the annual fee?
No. Most insurers view the wellness services encompassed by the annual fee as non-covered services and do not provide reimbursement for the annual fee. However the annual may be paid through most employer section 125 plans and is compatible with most flexible spending accounts (FSA’s), medical savings accounts (MSA’s), health reimbursement accounts (HRA’s), and health savings accounts (HSA’s).
Can I be seen if I don’t have Insurance?
Yes. Once you have joined the practice all visits are just $30 if you do not have insurance or it is out-of-network insurance.
How do I get refills of my medications?
To get a refill of any medication, please call your pharmacy and have them fax or send an escribe request. Once we receive the request, it takes 48-72 hours to review and process medication refills. Please be sure to request refills at least 3-5 business days in advance so you do not run out of medication. Controlled substance request and refills please call to the office.
How do I get copies of my medical records?
To get a copy of your medical records, please fax a request or stop by the front desk and ask to fill out a records request. Please allow 2 weeks for all requests. A fee may be charged.
What should I bring to my appointment?
Please make sure to bring your insurance card, photo ID, and co-pay if it’s required by your insurance. If you are a new patient, please bring a detailed list of current medications or prescription bottles and the filled out New Patient Paperwork (see Preparing for Your Appointment under Patient Resources).
Do you accept my insurance?
We do participate with most major insurance companies. But it is the patient’s responsibility to contact their insurance company to verify if we are an in-network provider.
